Clinical outcomes of patients with hypothyroidism undergoing percutaneous coronary intervention

After a median follow up of 3 yrs patients with a history of hypothyroidism were shown to have a higher risk of major adverse CV and cerebral events, cardiac death, MI, heart failure, and target vessel revascularisation following PCI compared with euthyroid patients.

Additionally, findings suggest that adequate replacement with thyroid replacement therapy among patients with a clinical history of hypothyroidism significantly reduces the risk of major adverse CV and cerebral events (MACCE) compared with patients with inadequate replacement or no replacement (HR: 0.69, P = 0.005),

thus highlighting the importance of monitoring TSH levels and maintaining a biochemically euthyroid state.

The authors suggest that these finding also support a role for the routine testing of TSH levels in hypothyroid patients presenting for PCI as part of standard laboratory testing.
